But even with the diamond lead and NS taking their 2 d's, how should S then know to lead a club? I tried it 3 ways as S: continuing a d; playing a trump; and playing a spade. I could not figure out how I should know that a club is the only play to defeat the contract. Thanks!

I would lead the king of spades on the given auction. So I wouldn't find the killing lead.
B5 West overcalls 3♥. Should East raise it to 4♥ then? Does the answer depend whether North X (at the table) or passes?
I would definitely bid 4♥ over 3♥, no matter what what my opponent does. I would expect to make it, but regardless, we always further the preempt: 9 trumps to the 3 level, 10 trumps to the 4 level. Here partner has shown 7 so we have 10 trumps.
